Under the general supervision of the manager and medical director PPCP-MCH , the incumbent

60% Provides care and guidance, on a daily basis, to patient/family as per the individualized interdisciplinary care plan

30% Contributes to the multidisciplinary team assessment of patient / family

10% Provides psychosocial information to the interdisciplinary team by

identifying problems and possible solutions

Assists and guides the patient / family in the PPCP;Takes part in the preparing the individualized care plan as part of the interdisciplinary team;Organizes and promotes a range of educational, recreational, and therapeutic activities inside the PPCP following the individual care plans developed by the interdisciplinary team; Assists interdisciplinary team in implementing therapeutic activities according to the patient’s individual care plan;Works closely with the interdisciplinary team to support patients in achieving their individualized plans;Provides follow-up as necessary;Identifies needs for new services for the clientele and participates in their development;· Participates in ongoing departmental activities: staff meetings, staff development as well as committees mandated to promote the well-being of patients and their families, the staff and the institution;Observes policies and procedures of the MUHC / MCH /PPCP;Recognizes and refers medical problems to the nurse or physician;Documents and signs patient-related encounters professionally and legibly;Participates in the orientation of new personnel and social counselor students, within the scope of his/her responsibilities.

 Educational:

Diplôme de fin d’études collégiales (D.E.C.) avec spécialisation en techniques de travail social ou en techniques d’assistance sociale d’une école reconnue par le ministère de l’Éducation, du Loisir et du Sport ou tout autre champ de spécialisation approprié tel que, « Special Care Counselling »

Work experience:

Two (2) years of experience as a Social Counselor (Technicien en assistance sociale) or

any other pertinent field in an acute setting preferred

Prior experience with adolescents and /or work in a psychiatric setting considered an asset

Fluent in French and English, spoken and written- Excellent communication and interpersonal skills,-Independent decision making capabilities -Knowledge of child and adolescent developmental stages· -Knowledge of and interest in social and psychiatric problems in children /adolescents-Knowledge of behavioral interventions with children / adolescents / families-Able to work on an interdisciplinary team-Good analytical skills-Basic computer knowledge
