An Incomplete Grade may be requested only for serious medical reasons or other extenuating circumstances beyond the student’s control which prevented the student from attending school for more than 3 weeks.
Complete medical documentation, including the reason, the length of the absence, the doctor’s name and number must be provided to the Registrar who will make the decision to assign an IN (Incomplete) remark to the course.
Click here and print the request form. After the form has been completed, submit it to the Registrar’s Office, H-Building, 1st floor. The request must be submitted prior to the end of the semester in which the course is taken.
